Marin Community Clinics Complex Care Team of Social Workers, Nurses, and Complex Care Navigators, are available to support individuals who meet criteria for our programs with complex medical and behavioral health conditions. The team assists individuals in navigating complicated systems of care through health coaching, care coordination, advocacy, and emotional support. Our goal is to help Patients become independent and confident in managing their own healthcare. For a referral to Complex Care, please talk to your provider.
