- Specialty services, such as behavioral health, may only be scheduled by referral from a Primary Care Provider.

If you are already a patient and have a login ID, you may log in to MCC Connect – our Patient Portal, to request a non-urgent appointment.
If you are having trouble logging in please contact IT Technical Support at (415) 755-2575. Calls are answered Monday through Friday, 8:00 a.m. to 5:00 p.m. After hours, please leave a voicemail message.

Making the Most of a Phone or Video Visit
Before your visit, please have your prescription medications.
For video visits, please have a Wifi connection and device with a camera.
If you are the parent or legal guardian of a patient who is a minor or a dependent, please make sure the patient is present.
Please turn your ringtone on and be in a private, safe space with little distractions (ex: avoid traveling in a car)
Please be available 30 minutes before or after your scheduled appointment time, and note that call may come from a blocked caller ID
